The team convened in August, 1993, and concluded its work with this report in February, 1994. The team interrogated all available casualty data bases and reviewed relevant professional literature and narrative accident reports prepared by the Coast Guard and other agencies for casual factors and recommendations. The team adopted a working definition of the Houston Ship Channel that coincides with the Vessel Traffic Service Houston/Galveston Area.
