Abstract
The team was established by Commander, Eight Coast Guard District, to develop "an iterative process that will assist planners, regulators and users to transform the Houston Ship Channel into a safer operation today and in the future." The team convened in August, 1993, and concluded its work with this report in February, 1994. The team interrogated all available casualty data bases and reviewed relevant professional literature and narrative accident reports prepared by the Coast Guard and other agencies for casual factors and recommendations. The team adopted a working definition of the Houston Ship Channel that coincides with the Vessel Traffic Service Houston/Galveston Area. This area includes the Houston Ship Channel, portions of the Gulf Intracoastal Waterway, the Texas City Channel, the Galveston Channel, the Bolivar Roads Channel, the Inner and Outer Bar Channels, and the Galveston Bay Entrance Channel.
